Adult Client: We will need all of the information completed by you and signed.

 

Minor Client Over 13: The client’s information needs to be completed and both the guardian and client need to sign the form. This is easier if there is a seperate email provided for the client and the guardian.

 

Minor Client under 14: The client’s information needs to be completed and the guardian needs to sign. The client is not required, but if they are old enough to understanf are encouraged to sign.
